Serving as a trusted liaison for patients and families by helping them navigate between specialists, hospitals, and home health services. I ensure that everyone involved in care is communicating, working together, and focused on the patient’s needs—creating a seamless, supportive experience and continuity of care. 

Reviewing prescriptions and treatment plans, simplifying instructions, and guiding patients and families so they feel informed, supported, and confident in making decisions about their care. 

Providing support before, during, and after hospital stays by reviewing care plans and discharge instructions, answering questions, and coordinating follow‑up care to ensure patients and families feel prepared, supported, and confident in the next steps.

Offering compassionate guidance to patients and families during difficult transitions, while ensuring dignity and clarity in every care choice. I provide unbiased referrals to trusted hospice services, act as an advocate to confirm that hospice teams are fulfilling their responsibilities and help families maximize the benefits and resources available to them.

Acting as a bridge to ensure patients and loved ones feel informed, included, and empowered in every step of care. Facilitating communication with family members who may live farther away will help them stay connected and actively involved in important decisions and updates.
